What is Sciatica?

Understanding the Sciatic Nerve

The sciatic nerve provides motor function to the hamstrings and lower extremity adductors, as well as indirect motor function to the calf muscles, anterior lower leg muscles, and specific intrinsic foot muscles. Its terminal branches also contribute to sensation in the posterior and lateral lower leg and the plantar aspect of the foot.

Sciatica typically arises from an inflammatory condition that irritates the sciatic nerve, causing radiating pain, weakness, and numbness through the hips and legs. Symptoms lasting for eight weeks or less, is known as acute sciatica. In chronic sciatica, the symptoms last for more than eight weeks.

Risk Factors for Sciatica

According to Miami Neuroscience Center (Sciatica Symptoms, Diagnosis & Treatment | Miami Neuroscience Centre, 2019), major risk factors for Sciatica include:

  • Aging: Bone spurs and bulging discs increase sciatica risk.

  • Occupation: Heavy lifting, prolonged standing or sitting, repetitive back twisting, or extensive driving.

  • Diabetes: Increases nerve damage risk.

  • Menopause: Osteoporosis during menopause can degenerate vertebrae.

  • Prolonged sitting: Sedentary lifestyles increase sciatica risk.

  • Genetics: Conditions like scoliosis can result in sciatica.

Ayurvedic Treatment for Sciatica: A Natural Approach

Conventional medicine typically treats sciatica with medication, physical therapy, and heat/cold therapy. In some cases, surgery may be necessary. However, #Ayurveda offers a specialized and alternative treatment for sciatica that is non-invasive, simple, and therapeutic in the long term.

Kati Basti: A Key Ayurvedic Treatment for Sciatica Relief

Kati Basti is a unique Ayurvedic technique that provides support for pain management and improves sciatica pain over time. "Kati" refers to the lower back, and "Basti" means to hold or contain. In this treatment, medicated oil is retained on the lower back using a special technique.

How Kati Basti Works

A container made of black gram flour dough is shaped like a doughnut and placed on the lumbosacral area. Warmed therapeutic oils, such as Mahanarayana, Dashamula, Kothamchukadi, Sahacharadi, Valiya Prasarini or other therapeutic oils, are poured into the dough, creating a pool of oil on the affected area. The temperature of the oil is maintained for 30-40 minutes (Verma & Sahoo, 2022; Prakash, 2017). After the process, the dough is removed, and a light massage is given.

Kati Basti is an effective external treatment for sciatica when performed for 3-5 sessions continuously within 7-14 days (Nirmal Bhusal et al., 2017; MD Haidar & Md Tanzil Ansari, 2021).

In Ayurveda, sciatica (Gridhrasi) is often attributed to an imbalance of Vata dosha, particularly in the lower back region. Vata governs movement, circulation, and nerve function, and when aggravated, it can lead to pain, stiffness, and inflammation.

How Kati Basti mechanisms work?

Kati Basti helps to pacify aggravated Vata dosha through several mechanisms:

  • Snehana (Oleation): The warm therapeutic oils used in Kati Basti have a soothing and lubricating effect on the tissues, which helps to counter the dryness and roughness associated with Vata imbalance. The oils penetrate deep into the tissues, nourishing and strengthening the affected area.

  • Ushna (Heat): The application of heat helps to improve circulation, relax muscles, and reduce pain. Heat also helps to liquefy Ama (toxins) that may be obstructing the flow of energy and nutrients in the area.

  • Marma Therapy: Kati Basti indirectly stimulates Marma points (vital energy points) in the lower back, which helps to balance the flow of Prana (life force) and promote healing.

  • Nadi Shodhana (Channel Purification): By improving circulation and removing obstructions, Kati Basti helps to purify the Nadis (energy channels), allowing for the free flow of energy and information throughout the body.

The specific herbs used in the therapeutic oils also have properties that are beneficial for sciatica. For example:

  • Mahanarayana Oil: Contains herbs like Ashwagandha, Shatavari, and Bala, which are known for their anti-inflammatory, pain-relieving, and rejuvenating properties.

At REVIVE Ayurveda, I had the privilege of treating a client with acute sciatica. She experienced numbness and stiffness in her lower right leg, leading to foot drop. She was hospitalized and diagnosed with a herniated disc around L4 and L5, which was pinching the sciatic nerve. Her profession as a carer in a nursing home, requiring long hours of standing and heavy lifting, contributed to her condition.

After being discharged from the hospital, she sought alternative treatment options, including Sciatica treatment in Ayurveda at REVIVE Ayurveda and Acupuncture.
